We are looking for an RN to perform site visits for our clients on the Medicaid In Home Support Services (IHSS) program. These visits involve visiting the client's home during a caregiver's shift, reviewing the client's care plan with the client and caregiver, identifying if any changes need to be made in the care plan, and certifying that the caregiver is proficient in the performance of all IHSS-related tasks. There are a few forms addressing these requirements that need to be completed.

Qualifications include at least two years as a licensed RN, preferably but not requiring familiarity with the Medicaid IHSS program, administrative thoroughness, and strong inter-personal skills. Training will be provided to ensure appropriate understanding and completion of the forms involved.

The pay for each visit, which should not require more than an hour to complete, is $50. There will likely be 4-6 visits per month but that can vary depending on the rate of client intake.

Job Summary
The Personal Care Provider provides unskilled personal care and household services for the client in their home. This person is integral in serving as a companion as well as ensuring our clients’ space is clean, safe, and organized.
 
Responsibilities 
  • Observes and maintains home environment to ensure the safety and security of the client;
  • Assists with household chores including cooking, meal preparation, cleaning, and laundry;
  • Assists in completing activities such as shopping and appointments outside the home;
  • Provides companionship including, but not limited to, social interaction, conversation, emotional reassurance, encouragement of reading, writing, and mind-stimulating activities;
  • Performs or assists the client with personal hygiene, including bathing, hair care, mouth care, and dressing, when skilled skin care, skilled transfer, or skilled dressing is not required.
  • Maintains timely service records of home visits;
  • Attends and participates in appropriate client care conferences, staff and education meetings, and agency committee meetings.
 
Qualifications
  • Valid current RN License
  • Valid, current driver's license and access to a reliable automobile, or
  • Access to public or other reliable transportation.
  • Completion of the appropriate twenty-hour training program, or
  • Successful completion of skills validation via direct observation of the performance of tasks
  • Two years or more personal care experience in a home care setting preferred
