Job Summary
The Scheduler Supervisor role is a health care position responsible for providing scheduling and supervision of Personal Care Services, personal care providers, and homemakers.
 
Essential Duties
  • Orients personal care and homemaker staff to the agency all policies and procedures while also completing the onboarding process for all staff.
  • Arranges and document training.
  • Schedules staff and notifies clients of changes.
  • Performs written assignments of duties on a client-specific basis.
  • Meets/conferences/educates with the staff as necessary.
  • Receives and reviews referrals of clients and follows up with them as needed.
  • Performs initial assessment visits to client homes, assigns caregivers, and completes the assessment and all open packet forms.
  • Performs supervisory visits to client's homes, each client at least every three months, or more frequently as needed for problem resolution, skills validation of staff, client-specific or procedure-specific training of staff, observation of client’s condition and care, and assessment of client satisfaction with services.  At least one of the assigned personal care staff must be present at supervisory visits at least once every three months per client.
  • Provides monthly supervision of client care or sooner based on client needs via telephone or other appropriate contacts with the client and/or family/caregivers.
  • Provides first-level investigation of complaints and incidents.
  • Counsels staff on difficult cases and potentially dangerous situations.
  • Communicates with the case managers, the physician, and other providers on the care plan as necessary to assure appropriate and effective care.
  • Oversees staff record-keeping.
  • Attends and participates in appropriate client care conferences, staff and education meetings, and agency committee meetings.
  • Maintains confidentiality in all aspects of job performance.
  • Adheres to, promotes, and holds others accountable for agency core values, ethics, and mission
  • Performs all job functions in compliance with all appropriate local, state, and federal laws, regulations, and professional standards.
  • Participate in an Administrative On-call Rotation.
  • Steps in and assists with client care/shifts when needed
  • Performs other related functions as required.

Qualifications 
  • A minimum of one year of experience as a Personal Care Provider is required.
  • Home Care supervisory experience is preferred.
  • Completion of a nursing program is preferred.
  • Valid, current driver's license
  • Access to a reliable automobile.
Compensation: $16.00 per hour
